Welcome to FORM — A Guided Journey to Becoming More Like Christ.
These Terms of Service (“Terms”) govern your access to and use of the FORM website, courses, community, videos, resources, and related services (“Services”) provided by Ken L. Roberts (“Ken,” “we,” “us,” or “our”).
By purchasing, enrolling in, accessing, or using FORM, you agree to these Terms. If you do not agree with these Terms, please do not purchase or use the Services.
1. ABOUT FORM
FORM is a Christian spiritual formation program designed to help followers of Jesus intentionally engage in the journey of becoming more like Christ.
FORM may include:
• Guided video sessions
• Teaching and reflection
• Practice guides
• Spiritual formation resources
• Weekly videos and reflections
• Community discussions
• Downloadable and digital resources
• Other materials made available as part of the program
FORM is an educational and spiritual formation resource. It is not a substitute for professional medical, psychological, legal, financial, or other professional advice or services.
2. FORM ACCESS OPTIONS
FORM COURSE — SELF-GUIDED
The FORM Self-Guided option provides individual access to the FORM course materials.
Self-Guided members receive:
• FORM course lessons
• Video teaching
• Practice guides and related resources
• Course materials made available through the designated membership platform
Access Duration: 12 months from the date of purchase, unless otherwise stated on the applicable checkout page.
The Self-Guided option does not include access to the private FORM Community unless community access is specifically included in the purchase offer.
Access is personal to the purchaser and may not be transferred to another individual.
FORM COURSE + COMMUNITY
The FORM Course + Community option includes the FORM course materials together with access to the FORM Community.
Members receive:
• Everything included in the FORM Self-Guided Course
• Access to the FORM Community
• Community discussions
• Community posts and reflections
• Additional community-based content
• Other resources specifically identified as part of the offer
Access Duration: The access period stated on the checkout page at the time of purchase.
The Course + Community option is a one-time purchase and is not an automatically recurring subscription unless otherwise stated on the applicable offer.
Community access is personal to the purchaser and may not be transferred to another individual.
3. INCLUDED FEATURES AND OFFER TERMS
The specific features, materials, bonuses, access periods, and other components included with your purchase are those displayed on the applicable checkout or purchase page at the time you complete your purchase.
We may offer different versions, promotions, bonuses, or packages of FORM from time to time.
A future change to an offer does not automatically change the terms of a purchase you have already completed.
4. COURSE ACCESS
After successful payment, or after receiving authorized complimentary access, you will receive instructions for accessing FORM through the designated membership platform.
You are responsible for:
• Providing accurate registration information
• Maintaining the security of your login credentials
• Keeping your account information current
• Using your account only for your personal use
You may not share your username, password, membership link, or course access with another person.
If we reasonably determine that account credentials have been shared or access has otherwise been transferred without authorization, we may suspend or terminate the account in accordance with these Terms.
5. COMMUNITY ACCESS AND CONDUCT
If your purchase includes access to the FORM Community, you agree to participate respectfully and responsibly.
You agree not to:
• Harass, threaten, or intimidate another member
• Bully or demean another person
• Share another member's private or personal information without permission
• Post unlawful, abusive, hateful, threatening, or inappropriate content
• Spam the community
• Disrupt community discussions
• Impersonate another person
• Promote unrelated products or services without permission
• Engage in unauthorized commercial activity
• Distribute private community conversations outside the community
• Share FORM course materials with people who do not have authorized access
• Attempt to obtain unauthorized access to another member's account
• Use the community for fraudulent or unlawful purposes
We reserve the right to remove content and suspend or terminate community access when we reasonably believe these Terms or community guidelines have been violated.
6. RESPECT FOR PERSONAL STORIES
FORM may involve conversations concerning faith, grief, relationships, leadership, life transitions, spiritual growth, personal challenges, and significant life experiences.
Members may choose to share personal experiences within the community.
Please treat those stories with care and respect.
You agree not to copy, record, reproduce, distribute, publish, or share another member's personal story or identifying information outside the community without that person's permission, except where disclosure is required by law.
Please understand that we cannot guarantee that every member will comply with these expectations. Do not share information within the community that you are not comfortable having other authorized members see.
7. PAYMENTS AND PRICING
The applicable purchase price will be displayed on the checkout page before you complete your purchase.
By completing your purchase, you authorize the applicable payment processor to process the transaction using the payment method you provide.
Prices, promotions, discounts, bonuses, and offers may change for future purchases.
Changes to pricing or promotional offers will not affect a purchase you have already completed, except as otherwise expressly stated.
You are responsible for providing accurate billing and payment information.
8. REFUND POLICY
We want you to have an opportunity to determine whether FORM is right for you.
30-DAY REFUND PERIOD
Eligible FORM purchases may be requested for a refund within 30 days of the original purchase date.

Please include the name and email address associated with your FORM account and identify the purchase for which you are requesting a refund.
REFUND PROCESSING
Approved refunds will generally be returned to the original payment method, subject to the payment processor's processing times.
Once a refund is approved and processed:
• Your FORM course access may be terminated.
• Your FORM Community access, if included with the purchase, may be terminated.
• You may no longer access or use FORM materials provided under the refunded purchase.
We reserve the right to review refund requests for suspected fraud, abuse, or repeated misuse of the refund policy.
Nothing in this refund policy is intended to limit any non-waivable consumer rights you may have under applicable law.
9. INTELLECTUAL PROPERTY
FORM and its materials are owned by or licensed to Ken L. Roberts, unless otherwise stated.
This includes:
• Videos
• Audio
• Course lessons
• Practice guides
• Written materials
• Graphics
• Logos
• Course frameworks
• Teaching materials
• Downloadable resources
• Website content
• Original community resources
Your purchase provides you with a limited, personal, non-exclusive, non-transferable right to access and use FORM materials for your own personal use during your authorized access period.
Your purchase does not transfer ownership of any FORM intellectual property to you.
You may not, without prior written permission:
• Reproduce FORM materials for resale
• Share course videos or materials with unauthorized individuals
• Upload FORM materials to another platform
• Sell, sublicense, or redistribute FORM materials
• Copy or reproduce substantial portions of the course for another program
• Share your membership access
• Use FORM materials to create or operate a competing commercial program
• Remove copyright, trademark, or other proprietary notices
All rights not expressly granted to you are reserved by Ken L. Roberts or the applicable rights holder.
10. PERSONAL USE ONLY
FORM is intended for individual spiritual growth and development.
If you would like to use FORM materials in a church, small group, organization, business, school, coaching program, ministry, or other group setting, please contact:
Individual membership does not grant a group, church, organizational, or commercial license.
11. USER-GENERATED CONTENT
If you post comments, reflections, questions, testimonials, photographs, or other content within the FORM Community (“User Content”), you retain ownership of your original User Content.
By submitting User Content to the community, you grant us a limited, non-exclusive right to host, display, and use that content as reasonably necessary to operate and provide the community.
We will not intentionally use private community discussions for unrelated public marketing without appropriate permission.
You are responsible for the User Content you submit and represent that you have the necessary rights and permissions to share it.
We may remove User Content that violates these Terms, community guidelines, applicable law, or the rights of others.
12. NO GUARANTEE OF RESULTS
FORM is intended to provide education, spiritual formation, reflection, encouragement, and community.
Individual experiences and results will vary.
We do not guarantee any particular spiritual, personal, relational, professional, financial, or life outcome.
Your experience may depend on many factors, including your participation, circumstances, decisions, beliefs, actions, and application of the material.
13. SPIRITUAL AND PROFESSIONAL DISCLAIMER
FORM provides Christian spiritual formation and educational content.
FORM is not:
• Medical treatment
• Mental health treatment
• Psychological counseling
• Legal advice
• Financial advice
• Professional business advice
• Crisis intervention
• A substitute for professional care
Nothing in FORM should be understood as a diagnosis, treatment recommendation, or professional opinion.
If you are experiencing a medical, psychological, legal, financial, or other professional issue, consult an appropriately qualified professional.
If you are experiencing an immediate emergency or believe you or someone else is in immediate danger, contact the appropriate emergency or crisis service in your location.
14. THIRD-PARTY SERVICES
FORM may rely on third-party platforms and service providers for:
• Payment processing
• Course hosting
• Community functionality
• Video hosting
• Email communication
• SMS communication
• Website functionality
• Analytics
• Security
• Customer support
Third-party providers may have their own terms, privacy policies, and practices.
We are not responsible for the independent acts, omissions, policies, or availability of third-party services, except to the extent required by applicable law.
15. ACCOUNT SUSPENSION OR TERMINATION
We may suspend or terminate your access to FORM if we reasonably believe that you:
• Violate these Terms
• Share unauthorized account access
• Distribute copyrighted FORM materials
• Engage in abusive or threatening behavior
• Engage in fraudulent activity
• Misuse the community
• Use FORM for unauthorized commercial purposes
• Engage in conduct that materially harms the Services or other members
Where appropriate, we may provide notice and an opportunity to correct the violation.
If access is terminated because of a violation of these Terms, any refund will be handled according to the applicable refund policy and applicable law.
Termination does not eliminate obligations that, by their nature, should continue after termination.
16. SERVICE AVAILABILITY
We make reasonable efforts to maintain the availability of FORM.
However, we do not guarantee that the Services will always be available, uninterrupted, secure, or error-free.
Temporary interruptions may occur because of:
• Scheduled maintenance
• Technical issues
• Internet or hosting problems
• Third-party platform interruptions
• Security incidents
• Events beyond our reasonable control
We will make reasonable efforts to restore affected Services when practical.
17. LIMITATION OF LIABILITY
To the maximum extent permitted by applicable law, Ken L. Roberts will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages arising out of or related to your use of, or inability to use, FORM or the Services.
To the maximum extent permitted by applicable law, Ken L. Roberts' total liability arising out of or relating to your use of the Services will not exceed the amount you paid for the specific FORM product or service giving rise to the claim.
Nothing in these Terms excludes or limits liability that cannot legally be excluded or limited under applicable law.
18. INDEMNIFICATION
To the maximum extent permitted by applicable law, you agree to defend, indemnify, and hold harmless Ken L. Roberts from claims, liabilities, damages, losses, and expenses arising from:
• Your violation of these Terms
• Your misuse of the Services
• Your violation of another person's rights
• Your User Content
• Your unlawful or unauthorized use of FORM
This provision does not require you to indemnify Ken for matters caused by Ken's own gross negligence, willful misconduct, or other conduct for which indemnification cannot lawfully be required.
19. CHANGES TO FORM
We may update, modify, reorganize, replace, or improve FORM materials from time to time.
Changes may include:
• Course organization
• Videos
• Practice resources
• Community structure
• Website functionality
• Supporting resources
We will make reasonable efforts to maintain the overall purpose and value of the program.
20. CHANGES TO THESE TERMS
We may update these Terms from time to time.
When we make material changes, we may provide notice through the Services, email, or another reasonable method.
The “Last Updated” date at the top of these Terms will identify the date of the most recent revision.
Your continued use of FORM after revised Terms become effective constitutes acceptance of the revised Terms to the extent permitted by applicable law.
21. GOVERNING LAW
These Terms are governed by the laws of the State of Ohio, United States, without regard to conflict-of-law principles, except to the extent applicable law requires otherwise.
Any dispute arising out of or relating to these Terms or the Services will be handled in accordance with the applicable laws and procedures of the State of Ohio.
Nothing in this section is intended to deprive you of any mandatory consumer protections or rights that cannot legally be waived.
22. DISPUTE RESOLUTION
Before initiating formal legal proceedings, you agree to first contact us at:

Please provide a description of the issue and the resolution you are seeking.
We will make reasonable efforts to resolve disputes informally before either party pursues formal legal remedies.
Nothing in this section prevents either party from seeking emergency or injunctive relief when permitted by law.
23. SEVERABILITY
If any provision of these Terms is determined to be unlawful, invalid, or unenforceable, that provision will be enforced to the maximum extent permitted by law, and the remaining provisions will remain in full force and effect.
24. NO WAIVER
Our failure to enforce any provision of these Terms does not constitute a waiver of our right to enforce that provision in the future.
25. ENTIRE AGREEMENT
These Terms, together with any applicable purchase terms, refund policy, privacy policy, and other terms expressly incorporated into the Services, constitute the agreement between you and Ken L. Roberts concerning your use of FORM.
If there is a conflict between these Terms and a specific purchase-page term applicable to your transaction, the specific purchase-page term will control to the extent of that conflict.
26. ELECTRONIC COMMUNICATIONS
By using FORM or providing your email address, you agree that we may communicate with you electronically regarding your account, purchases, course access, community participation, support requests, and other matters related to the Services.
Where required by law, marketing communications will include appropriate unsubscribe or opt-out instructions.
